Youth Sports Participant Form

This Agreement is made between Arborlawn United Methodist Church of Fort Worth, Texas (“AUMC”) as the owner and operator of certain facilities, namely the Family Life Center (“Facilities”) and the athletic participants and coaches (“Participants”). In consideration of the use of the Facilities for athletic practices, games, or tournaments, the undersigned Participant agrees to assume full and complete responsibility for any injury or accident which may occur to Participant or Participant’s property during his/her involvement in an athletic event at the Facilities or while present on the property. Participant hereby releases and holds harmless AUMC, its employees, members, or volunteers of liability in the event of personal injury or property damage. Participant agrees to conduct himself/herself in a manner consistent with Christian values and good sportsmanship. Participant further agrees to fully compensate AUMC for any property damage that Participant causes directly or indirectly to the Facilities as a result of his or her willful, negligent or reckless behavior.
